What advice can you give me about a setting goals for a nursing careplan?

I am having trouble developing goals for my nursing careplan. Is there any tips that anyone can give me. For example, risk for impaired skin integrity R/T drainage secondary to peg tube. What advice can you give me about a setting goals for a nursing careplan?
The best goals are ones that are achievable. So it is very important to critically think about what your pt. can do reasonably. Goals should contain a time frame and they should be written in a positive manner, the wording needs to be authoritive so use words like will rather than should, and it is a goal so it should alway be writtten in the present/future never in the past.





So you might say that


Pt. will keep skin around peg tub clean and dry (assuming the pt is cleaning this himself) otherwise you might say that skin around peg will be kept clean and dry.





What advice can you give me about a setting goals for a nursing careplan?
Patient will maintain current level skin integrity.


or


Patient will not have any skin breakdown for remainder of hospital stay.








You need to consider the current condition of the patient as it is related to the nursing diagnosis. The statement must be in the form of a positive patient outcome also in relation to the nursing diagnosis.
